Course Summary
New primary cervical screening guidelines from both the American
Cancer Society (ACS) and from the American College of Obstetricians
and Gynecologists (ACOG) include the option to screen women
age 30 and over with a HPV test plus cervical cytology. An understanding
of the natural history of HPV as it relates to cervical screening,
of the rationale behind combined screening, and of the recommendations
for women having these combined results is a prerequisite for
making appropriate clinical management decisions and for educating
patients about this sexually transmitted virus. The ASCCP CME
course titled "Primary Screening with HPV Testing and Cervical
Cytology Combined" provides the basis upon which clinicians
can decide whether to implement this new approach to cervical
cancer screening, and if so, guidance on management and on patient
education necessary to facilitate such screening.
The course opens with a
discussion of the relationship between HPV and abnormal cervical
cytology, CIN and cervical cancer, followed by an overview of
the general interval recommendations for cervical cancer screening
of both the ACS and AGOG. The course then proceeds into an in-depth
review of the data and rationale behind screening women age
30 and over with HPV testing combined with cytology. The next
section discusses the new NCI-ASCCP-ACS “Interim Guidance” recommendations
for managing women found to be HPV positive but Pap test negative,
and various other combinations of Pap and HPV test results.
Logistics of combined screening, methods for testing for HPV,
clinical parameters for use, and patient counseling and education
issues are also explored.
